Problems Setting Up ZenPhoto

I am having problems with the setup of ZenPhoto, I have uploaded all of the files and I have run the set-up, but not all of the elements for the set-up page appear. Here is what I end up with when I run the setup

I believe it is supposed to allow me to customize the config file from here to setup my database tables.
